Free Clinic Management Software in India: What You Actually Get (and the Limits)

An honest look at free clinic management software for Indian doctors: how free plans are paid for, what to check before you commit, and exactly what MedPrax OPD's free Starter plan does and does not include.

"Free" is the most searched word next to "clinic software" in India, and for good reason. A new practice has rent, equipment and staff salaries to worry about, and paying for software before you have a steady patient flow feels backwards. A free tool lets you get off paper without a monthly bill.

But free software is never really free for the company that makes it. Somebody pays for the servers, the support and the developers. Before you put years of patient records into a free product, it is worth understanding how that bill gets paid and what the limits are. This article explains what to check, and then lays out exactly what our own free plan includes, including the parts that are not free.

How free clinic software is usually paid for

Broadly, there are four models. None is automatically bad, but each has trade-offs you should know about.

  • Freemium. A free plan with limits (patients, visits, features or users), and paid plans for busier clinics. The company earns from the clinics that upgrade. The key question is whether the free plan stays usable or is quietly squeezed over time.
  • Free trial only. Full features for 7, 14 or 30 days, then you pay or lose access. This is not really "free software", just a try-before-you-buy period.
  • Advertising or sponsorship. The software is free because someone else, often a pharmaceutical company or a diagnostics brand, pays to reach you or your patients. Ask what is shown on your screen and on the patient's prescription.
  • Pay-per-use extras. The core is free, but SMS, WhatsApp messages or other features are charged per use. This can be fair, but the costs add up quietly in a busy OPD.

Six questions to ask before you commit

1. What exactly is the limit?

Is it patients, visits per month, prescriptions or users? A limit on visits per month is easier to plan around than a lifetime limit on patients, because your monthly volume is predictable.

2. What happens when you hit it?

Do you get a warning? Can you still see old records? Is the data held hostage until you pay? A reasonable product lets you keep reading your records and simply asks you to upgrade before adding more.

3. Can you export everything, on the free plan?

This is the single most important question. If you can download all patients and visits as a spreadsheet at any time, you are never locked in. If export is a paid feature, your data effectively belongs to the vendor.

4. Are there per-message charges?

Sharing prescriptions on WhatsApp is now expected. Find out whether it is free or metered, and whether messages go from your clinic's own number or the vendor's.

5. What appears on your prescriptions?

Some free tools add branding or advertising to the printed prescription. Look at a sample printout before you hand one to a patient.

6. Is the free plan likely to survive?

You cannot know the future, but a free plan that sits alongside clearly priced paid plans is more sustainable than "free forever" with no visible business model.

MedPrax OPD's Starter plan, honestly

Our free plan is called Starter. It is free forever, not a trial, and it is designed for a single doctor starting out or seeing a modest number of patients. Here is what is included and where the limits are.

What Starter includes

  • One doctor, up to 150 visits a month. That is roughly 6 patients a day over a 25-day month.
  • Token queue with auto-numbered daily tokens, call next, and waiting, in-consult and done statuses. See the OPD queue.
  • Full e-prescription: over 3,000 built-in Indian brands and generics (3,074 at the time of writing), your own medicine list, 1-0-1 style shortcuts, vitals, diagnosis, investigations, advice and follow-up. See e-prescription.
  • Picture dosage instructions and a second line in any of 14 Indian languages, so patients understand when to take each medicine.
  • Allergy and duplicate-medicine alerts while you write.
  • Print and WhatsApp sharing with no per-message fees, because the message goes from your own WhatsApp.
  • Patient IDs with a printable QR card, so returning patients are found in seconds.
  • Medical, fitness and referral certificates, billing with exact-amount UPI QR, and receipts. See clinic billing.
  • Export all patients and visits as spreadsheets, any time.
  • Works offline on signed-in devices. Data hosted on Google Cloud in Mumbai, encrypted in transit and at rest.

There are no advertisements in the app, on any plan.

What Starter does not include

  • No staff logins. Starter is a single login. If a receptionist will run the queue on a separate device, you need Pro, which includes two staff accounts.
  • No online booking page, waiting-room TV display or reports. These are on Pro.
  • No prescription templates or custom letterhead. Starter prints a clean prescription with your clinic details, plus a small "Generated with MedPrax OPD" line at the bottom. Pro removes it and adds your custom letterhead.
  • No visits beyond 150 a month. When you reach the limit, the app asks you to upgrade before adding the next visit. Your existing records stay fully visible.

When Starter is the right choice

  • You are setting up a new practice and seeing fewer than about six patients a day.
  • You consult part-time, for example a few evenings a week alongside a hospital job.
  • You want to try digital prescriptions properly before deciding to pay for anything.

When you will outgrow it

If you see 10 or more patients on most days, you will cross 150 visits within the month. Pro is ₹399/month, or ₹3,999/year (about ₹333 a month), plus GST, with unlimited visits. If you stop paying later, your account drops back to Starter. Nothing is deleted.

A sensible way to start

  1. Open the live demo. It sets up a sample clinic with patients, a queue and past prescriptions, with no sign-up.
  2. If it feels right, create your clinic. Every new account starts with a 14-day Pro trial, no card needed, so you can see the paid features too.
  3. After the trial, stay on Starter if 150 visits a month is enough, or upgrade if it is not.
